Mandatory Yearly School Registration Forms
|Each year EVERY Student must complete the Registration Packet & Student/ Parent Von PRIDE Values & Expectations Contract.|
CPS Registration Packet
The Registration Packet must be completed and turned in EVERY year by ALL Students no later than Friday Aug. 25th.
You can either print the Packet out and turn the signed copy into your division teacher or into 110. Or you can complete it electronically.

The Packet includes:
- The School Enrollment Form
- Family Income Form
- Emergency Contact Form (only those listed are allowed to pick your student up from school or receive communication about your student)
- School Messaging consent Form
- Race & Ethnicity survey
- Media Consent Form
If you are an incoming Freshmen you also need to submit the following documents, either in person to your division teacher or to 110 or electronically through THIS form:
- Birth Certificate (proof of Age)
- Parent or legal guardian State Issued ID card
- Two forms of proof of Address (proof of residency)
- Copy of IEP or 504 (if applicable)
- 8th grade report card
If you are a rising SENIOR, you must submit a copy of your immunization records showing you are up to date with your vaccinations including the Meningitis vaccine (also known as MCV4 or meningococcal ACWY). You can submit a copy of your immunizations either in person to your division teacher or to 110 or electronically THIS Form.
Student & Parent Panther PRIDE Values & Expectations Contract
Each year, Students and Parents are expected to carefully read through the contract together and then sign the contract. So everyone fully understands what is expected of them and what it means to be a Panther.
These Values and expectations have been established to create a safe and welcoming academic environment. Where ALL students and staff feel valued, respected and heard. And ALL are able to be their best selves in order to reach their full potential as they find their place in the world.

Directions on Completing the Electronic Documents
To complete the Registration Form Packet and Contract Electronically,
- Click the link(s) above, then click the Blue Button "Fill a Copy" on the top right hand corner.
- Then, sign in with a google account (or use your student’s CPS account)
- Or if you do not have a google account,
- Click - “Create a new account” with any email account
- And fill in the prompts
- Next verify through your email (it is FREE)
- And you will be able to use it for all of our forms moving forward
- Once you are signed in (either via google or your new account) go back to the document
- And click "Fill a Copy" one more time and follow the prompts and you are ready to go!
- Make sure you complete all of the required fields on the document and click the Green Button “Finalize” at the top and it will automatically send it to the school.
